Medications

Students who have asthma, diabetes, an allergy, or a chronic condition that requires or may require medication at school must submit the completed form to the school office. The form needs to be completed and signed by a physician by the first day of school. If your student has a rescue medication (EpiPen or inhaler) that they either self-carry or need access to during the school day, please make sure that you have completed the Authorization for Student to Carry and Independently Self-Administer Emergency Medication.

Required Vaccinations

The North Carolina General Statutes (G.S. 130A-152(a)) requires immunizations for every child present in this state. Every parent, guardian, or person in loco parentis is responsible for ensuring that their child(ren) receives required immunizations. If you have specific questions regarding your child, please contact your child's healthcare provider or your local health department. North Carolina requires the following immunizations for 7th and 12th grade students:

All 7th graders must present proof of immunization for a TDap Booster and Meningococcal Vaccines.

Students entering 12th grade will be required to have a booster (second) dose of the meningococcal conjugate vaccine if they have not already received one.

An up-to-date Immunization Record showing that your child has received the required immunizations is due by the first day of attendance at school.  If proof of immunization is not provided within 30 calendar days, your child will not be allowed to attend school until it is provided, as required by law. Proof of immunizations can be brought in to school for the Clinical Aid or emailed to vbrame@tmsacharter.org .

Physical Exams/Health Assessments

Starting in 2016-17, parents/guardians must submit proof of a health assessment for each child who is presented for admission into N.C. public schools for the first time. The health assessment must have been completed within 12 months prior to the date the child would have first been eligible for initial entry into the public schools. (General Statute 130A-440; 10A NCAC09.3005)

State-Required Health Information

North Carolina law requires our school to inform parents and students about certain health-related issues. We have included the text of the law below to help you understand your rights and our obligations. The website links listed below will direct you to additional information. We encourage you to have discussions with your students to help them understand the law, the issues, and the information.

NC General Statute 115C-218.75

Chapter 115C Article 14A Charter Schools 115C-218.75 General operating requirements. (a) Health and Safety Standards. – A charter school shall meet the same health and safety requirements required of a local school administrative unit. The Department of Public Instruction shall ensure that charter schools provide parents and guardians with information about meningococcal meningitis and influenza and their vaccines at the beginning of every school year. This information shall include the causes, symptoms, and how meningococcal meningitis and influenza are spread and the places where parents and guardians may obtain additional information and vaccinations for their children. The Department of Public Instruction shall also ensure that charter schools provide parents and guardians with information about cervical cancer, cervical dysplasia, human papillomavirus, and the vaccines available to prevent these diseases. This information shall be provided at the beginning of the school year to parents of children entering grades five through 12. This information shall include the causes and symptoms of these diseases, how they are transmitted, how they may be prevented by vaccination, including the benefits and possible side effects of vaccination, and the places where parents and guardians may obtain additional information and vaccinations for their children. The Department of Public Instruction shall also ensure that charter schools provide students in grades seven through 12 with information annually on the preventable risks for preterm birth in subsequent pregnancies, including induced abortion, smoking, alcohol consumption, the use of illicit drugs, and inadequate prenatal care. The Department of Public Instruction shall also ensure that charter schools provide students in grades nine through 12 with information annually on the manner in which a parent may lawfully abandon a newborn baby with a responsible person, in accordance with G.S. 7B-500.
